Based on 2009 discharges:

• 76 percent of Hillcrest Inpatient Rehabilitation patients went home

• 16 percent went to intermediate care facilities; and

• 7 percent returned to an acute care setting.

 The treatment team works with patients and their families to determine the best discharge environment based on individual needs at the time of discharge.
