Social Work Services

One person from the Hillcrest Community Hospice you will meet is a social worker. Hospice social workers are licensed by the State of Texas and have been specially trained in hospice care and grief management. On the initial visit, the social worker will complete a psychosocial evaluation to help assess the patient and family's needs and make them aware of available resources.

Other services include:

  • Explanation of service and insurance coverage.  
  • Team care plan for counseling, spiritual care, referrals, and volunteer services 
  • Discussion of legal issues including will, power of attorney, living will. 
  • Assessment of financial needs and assistance with referrals as needed.  
  • Discussion of caregiver plans and options.

The social worker will make a plan with the patient and family for subsequent visits and input as needed to determine if the patient and family would like a social worker to visit weekly, bi-monthly or monthly. Frequently, these will be short visits to give the patient and family an opportunity to review hospice care plan needs. Additionally, the following social work follow-up services are available for the patient and/or family:

  • Anxiety/depression management 
  • Grief management 
  • Life review for patient 
  • Stress management for caregivers 
  • Children's grief 
  • Relationships
